Community Care Options
If you, or your family, are facing financial difficulties, take a moment to talk with the Hopedale Medical Complex staff to learn more about Community Care Financial Assistance.
As part of the Hopedale Medical Foundation's charitable mission, no patient will be denied services solely on the inability to pay.
However, if the services are not emergency related, Community Care Financial Assistance is available if applied for prior to services being rendered.
Community Care Financial Assistance is awarded to those who apply and qualify based on the individual or family's gross income and available assets. The program is for individuals who do not qualify for other assistance programs such as public aid, and for those not already receiving the same or similar services.
Community Care does not apply to physician's services, cosmetic, and elective procedures.
What are the steps involved to qualify?
You must first apply for services that are covered by other agencies. The primary agency is Public Aid. If you are denied, you must provide Hopedale Medical Complex with a denial letter (as requested on the Community Care application). If you are unsure of the closest office in your county, ask your Hopedale Medical Complex financial counselor for help.
Public Aid requires the application to be made before services are provided.
In order to receive Community Care, you (or the Guardian/Power-of-Attorney) must fill out an application and submit it with the required attachments to the Hopedale Medical Complex Patients Accounts office.
You will need Adobe Reader to access the application. The link above provides you a resource to acquire the free software. Printed applications are also available at the Medical Arts Physicians' Office, Admission, Patient Accounts, Nursing Home, and Commons.
What next?
You will be notified as soon as possible of the amount of the financial aid assistance and a payment plan (if the assistance is less than 100%). Hopedale Medical Complex reserves the right to reconsider approval if we receive information that a family's financial situation has changed or false information was provided or omitted.
