How do I indicate my advanced directives?

If you already have an advanced directive, a living will, or healthcare power of attorney, it is important that you provide the hospital with a copy.  Hopedale Medical Complex will follow your wishes to the extent permitted by law.

Your attending physician should also be made aware that you have a living will or a Durable Power of Attorney for Healthcare.

What are Advance Directives?

There are three different documents that you (and your doctor) can complete that will direct us on how you wish your healthcare to be delivered if you become incapacitated.  They are the Living Will, Durable Power of Attorney for Healthcare, and Do Not Resuscitate “DNR”.  With these documents, you can convey to your doctor whether or not you want life sustaining procedures instituted if you develop a terminal condition or suffer sudden cardiac arrest.  It’s possible that you may want all three documents to ensure your wishes are followed.

If you have questions regarding Advance Directives, ask your nurse and feel free to discuss with your doctor.  Additional assistance in preparing these documents is available through the social service staff at 309-449-4381 or Administration at 309-449-4296.  There also is written information in your admission packet.

If I have questions or concerns during my stay, where do I direct them?
Concerns go to the patient satisfaction coordinator. Ask any patient care personnel to put you in touch with our coordinator.  General questions should go to the Department Head for that area.

When are meals served and do I have choices?
Meals are served in the hospital at 8am, 12pm and 5:30pm. Meal choices are available in keeping with your doctor’s orders.
